The application needs to be filled out to the best of your ability and returned to us as quickly as possible. The information you furnish will help us determine whether Potter is a suitable facility for your child. You will be contacted once your application has completed our review process.  The following documents are mandatory if your child/ward is to be placed with our agency. These documents include:   1.  Original Birth Certificate   6.  Medical or Insurance card and forms 2.  Social Security Card 7.  Current (6mo.)
